Adverse drug reactions to anthroposophic and homeopathic solutions for injection: a systematic evaluation of German pharmacovigilance databases

PURPOSE: Medicinal solutions for injection are frequently applied in anthroposophic medicine and homeopathy. Despite their extensive use, there is little data published on the safety of these products. Therefore, we investigated the safety of anthroposophic and homeopathic solutions for injection through a systematic evaluation of adverse drug reactions (ADRs).
"METHODS: ADRs were extracted from the pharmacovigilance databases of eight German manufacturers. Analysed ADRs included case reports in humans only, (spontaneous) case reports from post-marketing surveillance, literature and clinical/safety trials.
"RESULTS: Between 2000 and 2009, in total, 303 million ampoules for injection were sold, and 486 case reports were identified, corresponding to a total number of 1180 ADRs. Of all case reports, 71.8% (349/486) included ADRs that were listed (e.g. stated in package leaflet), and 9.5% (46/486) of the reports were classified as serious. The most frequently reported ADRs were pruritus, followed by angioedema, diarrhoea and erythema. A total of 27.3% (322/1180) were localized reactions for example; application or injection site erythema, pain, swelling and inflammation. The overall reporting rate of ADRs associated with injections was less than 4 per 1 million sold ampoules and classified as very rare.
"CONCLUSIONS: Our systematic evaluation demonstrated that the reporting rate of ADRs associated with anthroposophic and homeopathic solutions for injection is very low. Most reported ADRs were listed, and one quarter consisted of local reactions. These findings suggest a low risk profile for solutions for injection as therapeutically applied in anthroposophic medicine and homeopathy.

DOEL: In de antroposofische en homeopathische geneeskunde wordt vaak gebruikgemaakt van medische injectievloeistoffen. Ondanks de veelvuldige toepassing van deze producten zijn er weinig data gepubliceerd over de veiligheid ervan. Daarom hebben wij in deze studie de bijwerkingen van antroposofische en homeopathische injectievloeistoffen op systematische wijze onderzocht.
"METHODE: Van acht Duitse fabrikanten hebben wij databanken voor geneesmiddelenbewaking onderzocht op bijwerkingen van de injectievloeistoffen in kwestie. De gevonden bijwerkingen hadden betrekking op meldingen van bijwerkingen bij mensen, (spontane) meldingen vanuit post-marketing surveillance (toezicht en onderzoek nadat het geneesmiddel op de markt is gebracht), en meldingen vanuit de literatuur en klinische studies.
"RESULTATEN: In de periode 2000-2009 zijn in totaal 303 miljoen injectieampullen verkocht, en 486 meldingen gedaan van in totaal 1180 bijwerkingen. Van deze meldingen ging 71,8% (349/486) over bijwerkingen die bekend waren (bijv. genoemd in de bijsluiter); 9,5% (46/486) van de meldingen was geclassificeerd als ‘serieus’. De meest gemelde bijwerkingen waren jeuk, gevolgd door zwellingen, diarree en een rode huid. Een deel van de bijwerkingen (322/1180, i.e. 27,3%) betrof lokale reacties: roodheid, pijn, zwelling of ontsteking op de plaats van injectie of toepassing. De meldingsgraad voor bijwerkingen van injecties was minder dan 4 per 1 miljoen verkochte ampullen, wat geclassificeerd wordt als erg zeldzaam.
"CONCLUSIES: Onze systematische evaluatie heeft laten zien dat de meldingsgraad voor bijwerkingen van antroposofische en homeopathische injectievloeistoffen erg laag is. De meeste van de gemelde bijwerkingen waren bekend, en een kwart bestond uit lokale reacties. Deze bevindingen duiden op een laag risicoprofiel voor de therapeutische toepassing van injectievloeistoffen in de antroposofische en homeopathische geneeskunde.
